- Home Forward Flyer * Case managers may provide the flyer to consumers which includes a direct line to CHSP/Home Forward. The number can be used to consumers utilizing Home Forward services to connect with them about questions, concerns, or to provide recognition.
- For case managers only: Case managers working with a consumer receiving Home Forward services who needs to escalate an issue after the consumer has been unable to resolve it through the contact number provided in the flyer above may contact Adrianna Rickard at (971) 269-9001.
